I'm ready to either scale back a lot of my main site (busy, stupid) or invest in a local FSED. I'm in TX. I feel like I'm about 15 years too late to get into this, but the company assures me it is going to be profitable in no time. It's a new site. Do any of you have any insights, warnings, or assurances? Thanks

Google neighbors free standing EDs. Lots of them went broke.

FSED is a game of headwinds and tailwinds. Find a FSED operator that will give you reasonable buy in equity, Open Book, True ownership/voice, and history of navigating through the the downturn (1998-2022).

Find one that has a good operating history 5-10+ yrs, and ask them if they ever closed a site. If they did not, then a group you should consider. Good Groups are hard to get into unless you know someone. When they open a new spot, their current owners want spots.

Currently FSED has a nice tailwind, thus many are opening.

If you want to DM me the name of the group, i will give you my honest opinion. But No group should tell you this. Just step back and think about this statement.

Avoid groups that base their success on Gray medicine (drug/etoh rehab, ketamine, Post partum depression, blah, blah), in bed with med mal/billboard lawyers, break the law. Not that they may not be successful at least in the short run or even the long run but you would be in bed with people who are a throw away from putting your license in jeopardy.

Neighbors and FC went bankrupt because they were not able to navigate the headwind years. They were top heavy and top guys/PE screwed the working owners. FC and Neighbors are being rebranded by other well run operators.
 
Isn't CMS looking into significantly reducing the facility fees that would affect FSEDs?
Most private FSERs do not take CMS. It surely will affect hospitals more. Now the privates may peg their reimbursement to CMS rates but likely less. Hospitals will fight tooth and nail at CMS cuts so likely a small haircut so will let the 800lb gorillas fight this out with CMS.

Like with any business, it is hard to see 1 yr from today less 5 yrs. I have heard that the FSER model is dying or not viable for 10+ years so I don't base my decisions on what "could happen"

I remember my SDG discussed opening a FSER during the early days, and the old fogies could not stomach the risk. Jumped in 8 yrs ago and everyone I brought into the fold left the hospital within 2 yrs and some 6 months. Never look back and all of my old SDG docs are FIRE.
