300 hours clinical volunteering.
some research experience without anything significant to show for it.
200 hours non clinical volunteering (teaching people to read, etc.)
TA for 2 semesters in science course
50 hours shadowing
scribe for a year
a few other random ecs

should i expand on ec's by taking a gap year or just apply?

Your stats are strong and you have the proper ECs; clinical exposure, mild research, leadership(through teaching) and community involvement. I'd only recommend applying if you can put together a strong app, which means having good letters of rec and PS, ready to go by the end of June, o/w you aren't doing yourself any favors by applying by mid/late cycle.

Also, if you were to take a gap year, what's your plan.

No need for a gap year; start applying June 1. Aim for low-tier MD schools, all new MD schools and any DO program. The MCAT is < avg, but not lethal. MSAR Online is your friend, so apply strategically and smartly.
