Management of the Patient With Acute Disease X

This chapter will discuss the Emergency Department management of the patient who presents with acute disease X. Disease X has a long history with many interesting facets and is of particular importance to Emergency Physicians. The differential diagnosis for disease X includes every pathologic process known to man from acromeagly to Zenker’s diverticulum and possibly some veterinary diseases as well. Disease X can present in any of a continuum of severities ranging from asymptomatic to death. When faced with a patient with acute disease X therapy must be initiated as soon as possible. After initial stabilization has been initiated emergent consults should be obtained from several specialties which are not available at your facility.

I think I saw a patient with this. I am a lowly intern so my experience with disease X is limited, but in my patient it presented as a "scabies infection that has gotten in my blood." Patient became quite irate I wouldn't write a prescription for medicated shampoo. Do you think this have been a related disease?

Quite possibly. Hematologic scabiasis can be either a common or uncommon sequealae of disease X, or both. You should get a stat consult from entomologic dermatology and admit to the Level I Parasite Center.

I'm trying to code to a level 5 for lortabpenia resistant to NSAID therapy. Any hints?

Yeah, order lots of tests (bonus credit for ordering unnecessary stuff), use the word intractable a lot, be sure you carefully document 200 ROS items and take about 30 minutes doing so.

And then collect diddly 'cause their self pay anyway.

Take care,
Jeff
