So my thought after seeing this, (other than I want to burn my eyes) is
1: How long do you think this guy had to get to an ED and at least get some fluids before he exsanguinates
2: Is there any way he survives this without surgical intervention
3: If this guy was bleeding out in front of you and there are no surgeons within the next hour, what could you do for him. Foley balloon in rectum, pack it with gauze? Pray to Jesus?

I don't know, he doesn't seem to be bleeding that rapidly. Assuming he's got a nml H/H to start and is otherwise physically healthy he's probably got 6-8 units of red cells to give so something like 3-4 L of whole blood. I think he's got a couple of hours...not that I'd want to challenge that
What about a Blakemore tube? Yes, I'm aware they're intended for the other end (and although I've heard about the actual use of Blakemore's it has never been outside the context of an M+M conference) but it seems reasonable to have a go at it. Gauze packing's probably not unreasonable as well. I think he could totally get through this w/o surgery.

A couple thoughts..
1) that dude is going to be sh*tting hot fire for a while.
2) He's going to need a really strong abx to treat that infection that possibly could occur.
4) This going to make the radiologist's day.
3) How would you even begin to word his aftercare instructions. Use caution around glass objects.
