good luck, freaked self out after reading this website too much about people failing. this is a horrible website for people who are either insecure or neurotic. unfortunately, i am probably both. regardless, for AMG's you'll probably pass (19 in 20 chance) and for IMGs so will you (4 in 5 chance) so good luck.
I used a few variations:

previous episodes
treatments tried
for the rest I did the basics but always wrote out a skeleton on the side of my sheet so i wouldn't forget, just remember to ask re meds/allergies

I liked this more than PAM hugs foss (past med, allrg,meds, hospit, urine, gi, sleep, fam, ob, social, sex) b/c this was how I usually wrote my h&p's in med school so more easily translated to typing. regardless, a mnemonic for those who are unorganized or slightly dim (like self) can usually help to make sure you get everything checked off.

also there's this ON CALL IDIOT mnemonic for the phone interview
onset
number of times
consistency, color of diarrhea/vomiting, crying
associated sx (fever, cough, etc)
lethargy, listless
liquids (peeing?)
immunizations
diet, daycare
infections
ORS /pedialyte
travel
generally, you won't need these mnemonics, but I thought they helped to keep things organized.
also, i always asked re. mood / anhedonia for quick depression screen
one other tip...
do the Hx and summarize everything prior to the physical exam
(1) the SPs "want" to ask you their challenge question (will do so during hpi or summary)
(2) you can give initial impressions and tests then, and then say, I would like to do PE now and will tell you my findings and update these impressions (unless you run out of time)
(3) there are clocks in the room, but i usually just used the signal (Houston)
i don't know if i passed, but it felt a lot better than if i hadn't prepared. i think the test for those who are consistently good is probably easy. it's a straightforward test. contrived definitely. however, for those who haven't done a medicine rotation in a while or take FOREVER to do a H&P, the above are good things to have
it's a well run day, 12 pts is a lot (5,brk,4,brk,3), you feel tired by the end of things. just go through first aid CS and maybe, maybe if you have an extra $50 to burn for no apparent reason, the usmleworld step2CS thing, but that's probably overkill. just certain tests are useful, such as remembering murphy's sign, brudzinki, kernig, romberg, rinne, weber, psoas, obturator sign, tinel, phalen, and you probably won't use any of them except for murphy's maybe
also, don't read this website too much. it will make you go really neurotic. but i prob prepped much more for it, which was good for me and hopefully will serve my patients well.
