So my ex-girlfriend ( in the process of reconciliation ) is graduating with a MD in a few days, does anyone have any suggestions on what to get for a future emergency medicine resident?

Not even close. I am in Bioengineering. yeah I should have tried to get in 😀

Not a lot of stuff is good for EM docs since they don't have offices in the hospital or anything. You could get her a nice frame for her diploma (~$150.00) or a small travel bag for toiletries for those nights in the hospital (~$40.00)
