hahaha, did anyone watch tonights episode and see the size of the liver?!?! I couldn't stop giggling. It looked like a kidney. Who in the world is the technical advisor for this show?

You couldn't get a good view of the liver, but it is quite possible that they only transplanted one lobe of the liver. The liver can regenerate, so often times they only transplant part of it. Maybe that is why is looked small. I'm not defending the show, just throwing out a possibility.

Bingo! A number of the "top" general surgery programs have 2 mandatory years of research. Some of my classmates who are headed towards general surgery keep saying there are 4 such programs that are 7 years long. I know that Duke is one of them, and I think Hopkins and Harvard are as well, but I can't remember the other one.

UVA has a 7 year track and does UCSF.

The Show Sucked Last Night. I Really Hope Surgery Programs Dont Need Their Residents To Compete For Cases. Thats Ridic.
 
Did anyone watch the show this week? I was really confused by the case where the surgeons removed the left half of the little girls brain. Wouldn't a girl that old (5-6 years) have complications from a surgery like that (i.e. paralysis of her right body)? I know that a lot of the pathways could regenerate due to plasticity, but don't you need to be fairly young in order for that to take place? I discussed this with a few classmates and we all think the show really downplayed the surgery...

Well, I'm just a pre-MSI, so I'm not sure of the science, but I think she was only 2-3 yo if that matters.

That's a good question. I would think it would be a pretty big deal. But to tell you the truth, aren't these shows always full of errors? I stopped watching GA after the first show where a subarachnoid hemorrhage presented with no pain. Was anyone else bothered by this? Unless our neuro professor was wrong, just about any other type of hemorrhage could be painless but the subarachnoid space in innervated by V so next to an aneurysm, this is the "worst headache of my life" kind of headache. 🙄

I was bothered by the teenage girl on the show this week. What was she doing on the surgical floor??? Grey didn't know what was wrong with her...the girl hadn't told anyone yet. What surgery were they planning on during before they found out why she was there? The interns are so excited about doing surgeries I suppose they were planning on making one up for even if she didn't need it. Exploratory surgery is always a good idea, it makes the patient feel like you care and are doing something. Right?

(Yes I realize she did need to have surgery, but that was not until after Grey discovered her scars on her stomach.)
 
Yeah, I thought I would like trying to find out what is wrong with each patient, but its turning out to be all drama and relationships.

Anyone else suspect is was Rasmussen's syndrome? I'm not even in medical school but I show a video I've seen like 20 times to my high school students in which this girl has to have a hemispherectomy because of her seizures. It felt good to have a guess come out right.

Thanks for pointing out all the inaccuracies. I'll be looking out for more if I can put up with the c@@p.

This reminded me of a book (autobiography) of Ben Carson who is the director of pediatric neurosurgery at Johns Hopkins. In the book Dr. Carson talks about his work developing successful hemispherectomy procedures. He spoke of one very successful procedure on a 14 year old girl so I guess anything's possible. In fact, he mentions several "older" peds patients that are successful, and also claims that he can count his unsuccessful cases on one hand.

The patient was in the ER ("the pit") initially. They consulted surgery because she had belly pain with fever, at which point they discovered that she'd had surgery. They then admitted her to the surgery floor and then they ordered the CT.

I'm really curious to get this question answered too. Yes the child was around 2 yo but wouldn't there be some kind of obvious loss of ROM or worse (neuro) when half your brain is removed?

We actually covered a case like this back in the Neuroscience module a few months ago. What happens is exactly what they said in the show, after removing one half of the brain the empty compartment just gets filled up with CSF and if the surgery is done in very young children they actually do maintain function and can walk, talk, etc... because there's so much plasticity at that age. I think they said the girl on the show was 2 years old so it was possible. It was like watching my Neuro lecture replayed on TV, very cool. 👍
 
this show is terrible...how can you all watch it? it's so medically inaccurate, it's beyond funny.
 
hemispherectomy?? Are you serious? You do realize that general surgeons DON'T DO THIS SURGERY. The show absolutely sucks and should be cancelled.
 
If I remember correctly, the surgeons said that the 2 year old girl would have an exellent chance of having a normal life. I don't think they made any guarantees or gave any false hope, and the parents should have been more worried about the stupid anesthesiologist (no offense to any anesthesiologists out there!).

You do realize that Dr. Shepard (I think that is his name on the show) is a neurosurgeon?

For those of you who say the show is very unrealistic and you can't watch it because of that, that's fine. But please don't suggest that those of us who enjoy Grey's are ignorant, simple-minded, etc. Some of us use this device called "suspension of disbelief", and can be entertained while realizing the show is inaccurate/makes medicine seem overly dramatic/is overboard in its stereotypes/etc.

Exactly. I don't really care if it's accurate or not. When I watch tv, I watch to escape medical school. It's my little break away from studying. I really don't care if it's perfectly accurate or not, so long as it entertains me.

I second that! The only problem is, I usually watch the show with non-med students... the whole time they're asking me things like: do they really do that?
I always have to answer with: I'm only a first year, but I think blah blah blah.... or I have no friggin' idea, ask me in 2-3 years! :laugh:

it's not that there are a few inaccuracies...it's that the entire premise of the show is totally inaccurate. Nothing that goes on is realistic -- even ER is better. It's a show from fantasyland.

It was a case for the surgery dept. (specifically neurosurgery)
If you remember they excised the complete RIGHT hemisphere of the girl.

She is going to have big time ..neglect syndrome i guess..??

My question is this: how exactly does the hospital heirarchy work? Did the show get it wrong, or was I just confuseled?

I saw that show for the first time last night and it was absolute trash! I'm willing to suspend my disbelief a little bit, but I decided it was FAR too unrealistic around the time they were performing a super-secret surgury on the attending to resect a brain tumor they found on CT THAT AFTERNOON....while elsewhere in the hospital residents were not getting fired for performing an autopsy (which apparently they didn't know how to do) against the EXPRESS wishes of the family.

Oh they also lost me around the time the surgical resident dropped this gem "if you cut his optic nerve how long will it take for his sight to come back?" christ aren't you supposed to be a doctor or something?!

In all seriousness I actually find Scrubs to be the most realistic medical show on TV...

ER is far too serious and far too busy

House is interesting because it hits all those nice rare conditions you learn but think you'll never see...but the structure of that hospital is just too strange...what department is House in again???

Grey's is just rubish

Scrubs hits closest to home i think...

I Keep hearing this from just about everyone and I've also seen this on disovery health but as unrealistic as Grey's is or ER they're still entertaining. It's TV and that's all. Just TV.

I just hope I never get HEMATOMACHROSIS like the poor schmuck that got hacked up in the autopsy...
