If it's just since studying for Step 1, then it may be related to stress. With so much hanging in the balance, who wouldn't be stressed?
Hope everything works out - others will probably have other more helpful ideas, this is just my 2 cents...

i am a medical graduate. i was already in this competitive field! Actually i was on the Dean list of best medical graduate. i studied medicine in different language so it is pretty tough for me to learn all the subjects with English but i am certain with yours help i can crack these exams! I thank you with all my heart.

It's hard to concentrate when you aren't really in to the stuff you are supposed to be reading. There are a few thing that you can do to. The most important is to force yourself to study when you are supposed to. Make yourself pay attention to what your reading. And stop every once in a while and paraphrase all that you have just read, to catch yourself from inattentiveness. I do this and it really helps me keep my mind from wandering. It's hard, and I know alot of people will say that's a waste of time, but really it's not. You have to train your mind, to stay focused, and gain that discipline it takes to read all this stuff again. Also find a really good place to study, without any distraction. It's hard to study when there alot of other more fun things to do than sit and read about the ligament of the knee. So find that place. My father is the one who does this for me, every evening he goes Hitler on my ass, about how much I actually did that day, and why I was at the top of my game.
I think also it is important to have a realistic schedule. Be smart, understand how much YOU can do in a day, and make your schedule according to that. I feel that alot of us overestimate the amount of info that we can study in one day, and especially when we are trying to catch up. So that in the end it becomes this impossible task, and finally we get so discouraged that we stop. DON'T let it get to that point. Because once you loss faith in yourself it becomes impossible to study, and getting that hope/faith back is very hard!! Trust me I know this very,very well. Feeling bad about yourself, makes you totally unreceptive to study. You have to stay positive, or else you will fail. My advice, you is 1) really look at your schedule and assess your personality, and find out if what you have planned, can actually be accomplished in the time you have allocated for it. Then remake your schedule so that you aren't playing catch up. Finally most important, make sure in your new schedule that you have one catch up day per week, so that you never get behind again. This is most important because if you get behind again you will feel like crap, if you can't catch up to the new schedule. 2)Force yourself to pay attention, and study for totally time your are supposed to(this is the hardest to do!!!) 3)Find a good place to study.
I hope you overcome this, but remember this is a daily struggle, and it will not be easy. Be strong, and you will get thru it. Good luck!![/QUOTE

Dude, first thing you need to do is RELAX. (wax on, wax off. Wait, nevermind...)
I have a feeling you've been thinking about Step 1 a lot. If so, that's good. Now you need to act on your thoughts. Sit your a$$ down, a few hours a day, and start reviewing. Here's your game plan:
1) Pick a subject (let's say immuno).
2) Now pick a review book (let's say Medical Micro/Immuno by Levinson).
3) Figure out how many pages (Let's say ~90 pages total)
4) Divide that into # of days (Let's say 9 days)
5) ALL YOU HAVE TO READ IS 10 PAGES A DAY!
See, that wasn't so bad. Stand up. Clean off the dirt. And get ready to beat cr@p out of Step 1!
