Have you taken DAT before question

I am looking at the supplemental application for the UNLV and it says 'Have you taken your DAT yet' and for yes and no (date you plan to take test) options. And there are places underneath you can write the scores..

I took it once in 2012 August once before (I didn't have a lot of time to study and suffered from toothache and headache that day) and didn't do well. I am planning on taking it again on June 30th, 2014 and I am wondering how I should answer that question. I don't want them to think that I am submitting the secondary application with the score I have from 2012... because I am pretty confident that I will do a lot better on this time.

What should I put? Any inputs are appreciated!!!

Since you are taking the DAT early in the cycle, I would not apply until AFTER your scores are in. That way, there is no chance a school will go ahead and look at your application before your new scores have been verified.
