White wall or other neutral background. Think like a drivers license, passport, or virtually any other id
 
My undergraduate school has a very iconic building. Should I take my headshot with it in the background and slightly blur it? Or should I just take it on a white wall? This is what it would like like, but with a blue sky because it is summer now (pic found via google, but that's the building I'm talking about).

We can see where you went to school without the visual aid.
It can only come off as pretentious (I think... I don't recognize the building).

My UG too has a slightly iconic building.
 
Sadly, I probably won't recognize that one, either!
I wouldn't have recognized it a year ago, either.

Actually, that's not true. I probably would incorrectly recognized it as a different tower at my sister's college.
 
I took my photo in front of the statue of liberty with an american flag on my shoulders instead of a sports coat. Only got one acceptance. Most med schools are anti American obviously.
 
About half of the applicants we interviewed last year used a neutral background. The rest had outdoor shots or lab/classroom photos that were so tightly cropped that you couldn't determine where they were taken. YMMV but that might be best. Avoid trying to telegraph your location with your headshot. It is pretentious.

However, I do think that you can get a decent picture on a cell phone for a med school application. No need for a fancy camera but if you have access to one, go crazy.

Mine was from a semi-formal. The before photo of course. Don't over think it.
