HELP Is this an IA?

It sounds as if you were not the subject of an institutional action aside from a note in your file that you were accused of a misdeed but not sanctioned in any way. I'd say "no, this isn't an IA" but the best policy is to ask for confirmation from the school by sending the definition from the AMCAS instructions to your school and asking for a reply in writing (email).
That way you can CYA if anyone ever says you falsified your AMCAS application by failing to report an IA.

@LizzyM Thank you - I really appreciate the feedback. Will do. In the event that I do have to report this, would be a complete app killer?
 
Thank you all very much. Piggybacking on this question, I've heard that some schools like WashU require Dean's certification letters or something of the sort. Since there is an internal record, would this raise an issue if this "IA"/"non-IA" not reported on my application

This is why you send the definition of IA to the people who hold the records (Dean of Students for your college-- start there) and ask, to have a response in writing as to whether or not you have ever been the subject of an institutional action and if so, a description of the action. You won't be the first person to have asked this question.
