Have a DO LOR, have an M.D./Ph.D. LOR (PI, took research for credit), waiting on my orgo prof's letter.

The reason for my downward trend is because the transition from a sheltered home life to a university without a support system was very difficult especially as I left home at the same time I became an ex-muslim.
There were some family issues I was dealing with as well, my dad's declining health, my grandmother passed away, my car got stolen one semester, I also dropped a dresser on my foot and had a fracture in my toe at the start of a semester.
