Hemodynamic parameters for dummies?

So, I am gearing up for another run through the ICU and always have a few patients with swans. I have read the section in the ICU book and memorize the hemodynamic numbers, but I never really have a great grasp on the physiology of these numbers and how they change (ex. why CO/ CI drops in low volume states, the various changes with different types of distributive shock). Can anyone suggest a reference that explains things in a stepwise manner that is relatively easy to follow?

If you're up to it, you can try "Pathophysiology of disease" by Stephen McPhee. It's more of a "med school" read but it explains the concepts very well. The chapter on cardiovascular pathophysiology will give you the concepts you're asking for here, but it's also great for respiratory, renal, GI pathophysiology. Once you master the concepts, it's much easier to build new knowledge, and manage the patient well.
