how are the exams generated?

is there a bank of 2000 questions and your test is determined when you sit down at the computer? or are different test-sites given different bunches of questions and your test is generated from that sub-set?

i've found that everyone who has taken the exam in my area said that the test was just as hard as qbank (i always heard qbank was harder). what's going on?
