I am likely being neurotic asking this, but I would like to have some opinions. How bad is a single "w" on a transcript when applying to an MSTP program considering 3.9+ GPD (for both cGPA/sGPA).

My reason for withdrawing is underestimating the "standard of proficiency" in a Calculus II course. I underestimated just how much I would need to know and genuinely understand the material. My earlier math classes were less rigorous, so I became used to this. If I am asked why I withdrew during interviews, this would be my explanation. No excuses. I will be taking the same class the following semester, hoping for an A with pre-studying.

Thank you

Not to worry. The W will not decrease your chances at an MSTP acceptance, especially if you do well when you re-attempt the class.
