How competitive are physician-scientist training pathways for residency?

I know this is pretty niche, but I'm wondering if anyone has any idea how competitive these programs are, since they are only available at select top programs.

I am not an MD/PhD, but have many years of research experience (including full-time research between M3 and M4 years) and multiple first-author papers.

They are very competitive and very niche. These are more common in other residencies like IM, but there are a few. I did one, and also helped interview candidates every year. I'm not aware that we ever accepted a non-MD/PhD applicant.
