how do colleges take into account your university?

One thing to note is that a much lower percentage of the individuals from the state schools actually make it to med school application. This is only the top few percent of students, and often the science/premeds have higher grades than the university average. You also have to remember getting into IVY colleges is based a lot on competitive high schools. A single high school can only have so many people in each school, this is the same thing happening to the IVY med applicants.
 
My experience is that the MCAT correlates with the difficulty of the school and the major. Competitive top 10 school? You'll probably do better on standardized tests. Hence, higher MCAT scores. My experience is that there are a lot of 3.8-4.0 students at lower-tier colleges who struggle to score in the mid-20s on the MCAT. Hence, the MCAT keeps things on a playing field and accounts for the difficulty of the undergrad school.

Conversely, it also gives someone who came from a no-name college the chance to compete with Harvard, MIT, and ivy league graduates if they score really well.

So the MCAT serves as a check to ensure that those who went to "easier" schools don't gain an unfair advantage and it also ensures that those who went to no-name school receive the same recognition as someone who did go to a top 10 school.

I think it's fair to say that standing out at ivy schools is definitely harder but it's also fair to say that many of the ivy schools grade so that all the students appear to "do well" because they believe they've still put in effort that would have allowed them to stand out at other schools. Essentially, grade inflation.

But honestly, being in the top 10% at an ivy is so much different from being in the top 10% of another school but having a high GPA at an ivy is not necessarily so much different from having a high GPA from another school.
