How do I document clinical experience?

I used to work in a hospital as an NA 1, but I moved on to working in private home settings. Will the schools I eventually apply to just check the state certification registry for NA 1 or should I get other documentation that I have been working as an NA 1?
The documentation for the hospital is easy to obtain, but I don't know how I will get anything official showing that I have many more hours working as an NA 1 in private homes.
I am just concerned that I won't be able to add my experience as an NA 1 in the private homes because I do not know how to prove it. I didn't work at the hospital nearly as long as I have been working in private homes.

First of all, I really don't think they'll check up on you at all. You just put what you did and if they ask for clarification, you can explain. I wouldn't worry about it.

Also, considering the private home setting is paid, don't they keep track of your hours? Don't you get a paycheck? Voila your proof. For me, my only clinical experiences were shadowing and volunteering so I just kept track of those hours by writing them down in a notebook where I also logged all volunteer hours and service. You could ask for a LOR (if it will be an amazing one) from your superior and that would also provide proof if you were still worried about it.
 
The AMCAS application does ask for a contact for each work or volunteering experience. As long as you know someone who can verify that you did the volunteering you said you did, you'll be fine. At the same time, you're basically on the honor system here. There's no way a med school with 10,000 applicants is going to try to verify every one of the fifteen work and volunteer experiences listed on each application.
