How do I tell the school I want to go there?

I know this is probably a dumb question, but I am currently holding two offers and tomorrow is May 15th. I thought I'd get an email from the schools asking me to fill out a form about my final decision, but I never got anything like that except for the initial form asking them to hold my place. Do I just send an email to the dean now or something saying I want to go there? I'm confused. Thanks!
 
Any school you get accepted to makes the assumption that you will be attending until you send them an email stating that you are withdrawing you acceptance. So send an email to the school you don't want to attend stating that you are withdrawing and you do not need to send anything to the school you want to attend. Congrats on your acceptances.
