Probably some sort of letter or note from the dentist you shadowed stating you had done so for x hours.

There's a section in the AADSAS online application for this, however, an LOR from someone in the place you shadowed might not be a bad idea.

All places you volunteer at usually have sign in sheets and stuff which can be used for proof that you volunteered there. However, you don't need to provide proof or documentation of volunteer hours to dental schools. You are pretty much on the honor system. If dental schools do decide to check up on your work, though you better make sure your numbers are somewhat accurate. 🙂
