how do you approach this 2CS case?

chief complaint posted on door:
patient wants a doctor's note.
vitals are stable.

this threw me off because there is no physical complaint.

on initial questioning, pt says sometimes he feels lightheaded at work.

is this a psych case? suspect malingering?

how do you approach this one? do you offer to give the pt the note in the end?

obviously the case cant be perfect but you ask why he wants a doctors note..what he tells you is the chief complaint then ask when he gets light headed
1) related to activities..getting up...medicines....dehydration..any syncopal episodes..does he lift heavy objects..does he have seizers..what does he do for a living..anay diseases..such as DM, thyroid..does any drugs? then ask if any problems at work..ask about vascular stuff such as heart problems and such..if you cover those areas you should be fine!
