How do you indicate interest in DO/PhD?

It's at the school level. Probably on a secondary application.
 
I think it also depends on the DO/PhD. MSU's you need to specify on the secondary and complete a DO/PhD-specific section. Rowan's you apply for once you're actually there (after your first year I believe).
 
Sorry I havent been able to respond but I have been on vacation. Typically when school's send you secondary applications, they will have an option which you can check off for DO/PhD programs and you will have to fill out additional essays. I recommend you start looking at which schools offer dual degree programs first. It may be beneficial to get in touch with faculty/admissions in those schools to talk to them about the program. After submitting the secondary then, the process proceeds like any other application. My recommendation is that you have letters from research mentors ready for submission (preferably 2). The basic requirement for these programs is sufficient research experience (1 year +) hopefully with some form of presentation experience.

I applied to MSUCOM and PCOM dual degree programs. I think Rowan has one also. I did not get invited for PCOM but was accepted into MSU. I believe MSUCOM has a very strong DO/PhD program with great financial support. If you are interested shoot me a PM.
