How do you know you've made it to the top in medicine?

When the people following you are walking at your pace.

I'm not a fast walker, but in the hospital, when I'm following around my attending or resident I have to keep pace with him. I'm waiting for the day when I can set the pace. 🙂

You've made it when you can make highly educated, well trained individuals from wealthy backgrounds fetch you coffee, and yell at them for not putting in enough cream (and get away with it).

...when you save a life, when you prevent major health problems from occurring in Pt.'s. That's when you've made it.
 
When you're the one that gets sued by the family who's kid you just saved. That's when you've made it.

When you are no longer the one getting the coffee or lunch for everyone. Or putting away everyone elses notes.
