How do you see your career in 10 ys?

I have a question when this ask this "how do you see your career in 10 yrs from now". As of right now, I'm not exactly sure what specialty I want to go into. I have some in mind, but they're SO completely different from each other...pediatric, int med, ophthalmology, or general practice. So for this type of question, would it be okay if I just choose one specialty to focus on and say that's what I'm going into, even though I'm not 100% certain?

i plan on just getting down with my general surgery residency in 10 years but i don't know if i will be going into a fellowship after that or not...

I'm wrtiing this essay as if i am already in 10 years. So everything i say about medical school/residency will be past tense and fellowships in future tense.
