How good is this LOR?

Hi,
I am an IMG applying for psychiatry residency this year. I am presently helping a Professor with his research in a University Hospital.
Recently I met a psychiatrist ,who is involved in research in a very big private research setting. He is willing to take me as a research assistant (full time) from November 2007( when I will be done with all my steps and will be free to assist him full time.)
And this psychiatrist is ready to give me a Letter of recommendation stating that he had interviewed me for research assistant position, and he is truly convinced about my interest in psychiatry,my English proficiency and communication skills etc. and he is willing to hire me as a research assistant from November 2007.
can I use this LOR as a 4th LOR for my application process?.Will this do me any good?.Or will it go in a negative way? This psychiatrist is from my home country.
Any input highly appreciated.

That's not a letter of recommendation, it's a personal letter of reference.

After your externship/observership he can access your clinical/technical know how and skills, that's an LOR.
 
That is what I am asking.
If I upload that Reference Letter to ERAS now, Will it do any good to my applications?
 
Most programs ask for three LOR. You could include this as a fourth, I don't see any negative outcomes from using this letter.

It may help you when applying to research oriented programs/program directors. Since these places like applicants who are have some experience in research.
