How many LOR?

At the very least you should have 3 letters. Some schools want only 3 and won't take additional letters. Some schools recommend that you send more than 3. Of the 3 letters, you should have 2 from science professors, 1 from non-science. You should not ask for the letter from a TA, unless the TA was the actual teacher of the course (even then, it would probably be better to ask for a letter from a prof if at all possible). If you've been involved in a steady kind of research/volunteer/work position through college or after college (if you've taken time off) you could use a letter from this experience as an extra letter. Otherwise, you could get an additional letter from a professor.

Some people will load up on letters and send 5 or 6. Personally, I think the quality is much more important than quantity. You want solid, personal references who can say more than "Applicant X got an A in my class". 3 stellar recs will get you farther than 5 form letters any day.
