How many years of research would be competitive for MD/PhD programs?

One year, obviously, is not enough. In addition, how significant must ones research be? For example, is it expected that you present a poster at a conference? Have a first-author publication under your belt, etcetera?
 
2+ years. No publications/presentations necessary, but helpful. LORs from your mentors and your understanding of and ability to discuss your research are most important, as is independence in your project(s).
