Just adding a Standardized Letter of Recommendation to my application in ERAS.

How did you handle documenting the "Author Name" and "Auther Title/Dept" for a standardized letter??

What exactly do you mean "handle documenting"? I thought it was pretty straightforward....put their name, put their title and which department they are in. Could you be more specific?
