How to do an opioid rotation from methadone

We have a patient on methadone 80 mg/day and Percocet 5/325 5 tabs per day and we wanted to do an opioid rotation to another long-acting agent such as Oxycontin, fentanyl or Ms Contin. What is the best method to do this?

I don’t mean to be condescending or insulting, but I am just curious.

Are you fellowship trained in pain?

Slowly decrease methadone each month until patient is off. Add a long acting medication such as Butrans or Belbuca. Done. Now you’re below the CDC guidelines. One of two things will happen... the patient will thank you one day or they’ll disappear to another candyman in town.

Did you really also just post to thread questioning whether patients on opioids can drink alcohol? Again, I think you are in the wrong forum.
 
80mg a day is not a pain-level dose. Recommend wean before changing any meds. Only med I would change to is suboxone.
