Like the title says..what's the best way to go about this? I imagine having visibility on the distal will be hard, and I won't have an assistant either to keep my mirror dry.

Have the patient close slightly and deviate the jaw to the left. Then overload the final crown with cement so periodontists actually have real work to do.

It can be especially challenging if they are heavier than the normal patient and have thick soft tissue/cheek in the way. I like to pop a dry angle in there, close slighty, rotate head to the right, deviate jaw to the left. Make sure you do the occlusal reduction first. Once you do all this it really isn't as bad as it seems. Always easier if there is no #16.
