what would be the best way to address academic concerns in a letter of interest, update, or intent?

Who has concerns and what are they? Are you concerned with your academic performance to date? academic performance since submitting your application? Pre-requisites for matriculation into a specific medical school?

Have you interviewed already?

You sorta asked this question before while you were applying... perhaps before. Can you give us more context to what has changed? Have you done any networking and received advice from admissions professionals?

None of these is an appropriate way to address academic concerns.

Hard to answer without knowing what your actual GPA and academic situation is, because it looks like you applied to schools like Harvard and Hopkins, etc. It may be that your school list was just top-heavy for your stats. If on the other hand your GPA truly is low, then nothing you write will convince a school that you can handle the academic rigor of medical education--you're going to need to put in the hard work to SHOW that you have reinvented yourself through a post-bacc or an SMP.

Bottom line, you are competing against many other highly qualified applicants. Your stats are your stats, and regardless of why you got that GPA you need to act accordingly. There simply is no reason for a school to take your word for it that you are better than your GPA.

Did this come up in an interview? If you were interviewed the school already knows about “ your academic concerns”. But really, idontunderstand your question. Sorry.
