Criminal Offense/being on sex offender or elder abuse registry
Bad LOR
Coming off as rude/unprofessional in essays or interviews
Going to the interview drunk/high or reeking of cigarette smoke
Zero volunteering and/or clinical exposure
Essays full of grammatical errors/typos
Caught lying on your application
Giving up a previous acceptance
