How to otain the fingerprint card for California pharmacist application

I am trying to apply for the intern and pharmacist license in California, which both require fingerprint. I called the California BOP and I also emailed [email protected] to request the form. So for, no response at all. I lived in Florida, and I don't want to fly out there to do the live scan unless I absolutely have to.

Do anybody know where else I can get this fingerprint form? Thank you.

I emailed whatever the website said to email - there was no reply email but the card showed up in the mail within a week.
