How to properly ask for a letter of reference from professor

I was thinking about asking my former biology professor for a letter of reference (she loved me). I was going to just go right to her office and pop the question but is there a certain etiquette I should use?

Also I'm going to ask my adviser soon, but is the pharmCAS a headache to deal with? and don't you have to have your professor send the LOR to them?

Actually, PharmCAS sends them an email with a survey to fill out. Pretty easy.

No particular etiquette, just be polite. Professors get it all the time, they will know what to do.
 
The poster above is right, there is no particular etiquette. I e-mailed my professor to set up a meeting about writing an LoR. We had a good talk and he offered to write one, good stuff.
