How to request transcripts

Hi--this is probably a very stupid question, but how do I go about requesting transcripts? Is there something I need to do on AMCAS? Or can I go directly to my undergraduate institution's registrar website that processes transcripts and ask them to send in a copy of my transcript to AMCAS?

Also, do I need one transcript for each medical school I apply to, or just one for AMCAS?
