How to send transcripts with form?

How do I send the transcripts from each institution with the "transcript matching form" enclosed with it? I am asking because the schools I need to send transcripts from have an online system for sending transcripts, you just order it, designate where you want it sent, pay and that's it - but according to the PTCAS instructions we have to get them to send it with the matching form enclosed? Does anyone know how to go about doing this?

My school works the same way with how you order your transcripts. In the PTCAS application instructions it says to contact the office that sends your transcripts and work it out with them in order to have the form sent with your transcript. However, I haven't personally called my university's admin office yet (I'm on the other side of the Pacific currently so it's not as easy to do) so I'm also curious about the best way to do it.

Speaking of being on the other side of the world -- does anyone know if the forms can be faxed (or does it require your signature?)?
 
For some universities (like mine) if you order your transcript online there is a place where you can add attachments if something needs to be sent with it. So I'll just save the transcript matching form to my computer then attach it online while ordering my transcript. My school will print both and mail them. You may want to see if yours has a similar option!
 
What I did last year was called each institution (three total) explained my situation of needing to include the document with my transcript; they ended up having me fax it over to take care of the rest of it. I had no trouble after that. Hope this helps and good luck!
 
After reading the responses I just checked my main university's online transcript ordering service and noticed that it does allow for attachments to be included, which is a great relief! However, my community college does not allow for attachments, but now that I know it's the kind of document that can be faxed (or emailed?) then it really takes away a lot of my stress and worry. That's one less call to make from overseas and two less things to have sent out...
