How to take a good history in 6-8 mins?

I'm having difficulty getting the pt's history in that time frame. Any tips on how to do that? I know it should be focused but I still feel like I'm taking a long time. Its hard to ask open ended questions with a study partner since the partner won't know how much to share. So we usually stick yes/no questions. Any advice will be appreciated.
