Hi, I would like to ask everyone's help regarding a survey I am doing. Thanks in advance...

1. Do you believe that there is such thing as anesthesia awareness?

2. Did you have a patient that suffered anesthesia awareness? If so, what type of surgery? What type of anesthetics used?

3. JCAHO issued a sentinel event alert last year. Does your place of employment have protocols/guidelines/educational program regarding awareness?

4. Do you use BIS to monitor awareness? Do you think BIS helps reduce/prevent awareness?

5. Years as an MDA?
