I need professional help on my application!!! HELP!

I'm a senior at a local SUNY college. I'm majoring in bioscience and getting a minor in chemistry. I'm thinking about either applying this year or next year.
I have shadowed a doctor and done volunteering at a hospital for 250+ hours over the course of 1.5 years. I plan on doing research at my school this upcoming semester.

I went to a 2 year community college and didn't do so well because I was going through a lot of family problems and couldn't focus on my education.

I have 1 more year left. I'm signed up for 17 credits this semester.
I plan on taking my MCAT this summer and aiming for 37+.
At the end of this year when I graduate and get all A this year... I will have a gpa of 3.44 in science and over all.

What are my chance in getting into a good MD school if I get above a 37 in the MCAT?
Should I apply this year or next year? Do medical schools look at the performance and how I excelled? I'm nervous and afraid. Please help me. I need guidance.
 
PS. If I get a W in Calculus 1, Organic Chemistry 1 & 2... My science GPA will go to 3.68 and my overall GPA will go to 3.67.
I'm retaking organic chemistry 2 this spring and assuming I get an A should I try to get W in them from previous school?
 
Your retakes will count towards your cumulative GPA for md schools. For DO, your retake grades can replace the previous ones where did poorly. Getting a 37 on the mcat is hard... Hypothetically, if you do get a high mcat and your ECs are solid then I think you'll have a good shot. Also your upward trend looks good to adcoms. GL.
