If anyone has saved the "community service" pdf form for OSU, could you please share it on here?

The link to that file on OSU's website has just been popping up a window that says, "file is damaged and could not be repaired"

itried to attach it here but I dont think it worked however I would try the link on the website again because I think they want you to fill it out on the computer and then print it out so that everything is clearly typed.
