Are we absolutely required to pass the IM Board Certification Exam after 3 years of Internal Residency to practice in New York for the next 30 or so years ?

Is it feasible in terms of insurance reimbursements and hospital privileges ?

Also, how does this IM Certification Exam compare to the Step I, II, III ?

You are not required to pass it. In fact its dependent on the hospital you work at or are affiliated with. Most places will in fact hire you with the intention that you will become board certified. If you choose not to, it does affect things as hospital privileges, reimbursement etc...
I am not sure about this but I think if you choose to go into solo private practice it doesnt matter at all.
 
You do not have to be board certified as there is emplyment for those who are not.However it will be a big impediment in your career.Over the the past 10 years it has become an increasingly important qualification.It will impact insurance plan/HMO/PPO participation as well as reimbursment and other issues.Some plans now require board certification.The great majority of those who complete an IM residency do pass the test so if you go to a decent program it should not be big worry.This same situation would apply to any specialty.
