Include former name if no current documents reflect it?

Members don't see this ad.
I changed my last name (not from marriage) several years ago so all my current documents are under the changed name. Do I need to include my former last name as well?

A lot of things will ask for former name.. especially a background check if you get accepted

I see. I interpreted this "Please add any names that may appear on transcripts, MCAT scores and prior AMCAS applications" as only applicable if my name was different on these documents. Don't you have to fill out additional forms for the background check once you get accepted and disclose it there? Or can they tell otherwise before your interview?

You will be asked this on the background check as well, and will need to answer this truthfully.

As to AMCAS, it does seem to only relate to matching documents on their end. That being said, the information is still transferred to medical schools, who may be unamused if a name pops up on your background check that wasn't initially disclosed. I would say to include it to be safe.
