I've lived on the East Coast for pretty much all my life, but I go to med school in the Midwest. Should I have two personal statements, one with a little discussion at the end about my ties to the East Coast and another for the Midwest, or is this implied by the hometown and school ERAS info?

I would include it in your PS if it's important for you to match in that region. If you don't care either way, it may not be necessary.

I did it because it wasn’t clear from my application why I would want to move to my desired location (my spouse’s home state). If you have clear ties, it’s not really necessary.
If you choose to have two statements just make sure you assign the right one to the right programs.

I included it. Why not? My school is a state from my home state.
