Is it about the same, specifically in an IM residency vs. IM fellowships?

You get a little more in fellowships because your gme/ogme year increases. As a resident your pay is somewhere between 40-50k and as a fellow it could be between 50-100k depending on your fellowship, location, acgme, or AOA. Best thing to do is look at specific programs

most, if not all, programs list their PGY-1 through PGY-8 salaries on web sites. in general, PGY-1 is 40-46, with incremental rises. fellowships are a continuation of that pay scale, unless it's not an ACGME approved fellowship(meaning Medicare doesn't fund it), so some programs often pay you more, like jr or adjunct faculty pay.
Example: GI pathology fellowships.
