Is it a good idea to tell schools on your interview day that you have interviews elsewhere? Or is the general rule to tell them only if they ask?

It's not their business. There would be no reason to inform them of this, even if they ask. It's your choice.

The reason I informed them is the following interview question:

Interviewer: What would you do if you didn't get accepted to medical school this year?

Me: I've already been accepted to AZCOM.
 
I got asked from a school that accepted me

I thought it was cool

It kind of showed they were interested.

If they ask, be honest with them. If they don't ask, you don't have to volunteer the information.
 
They know you are applying to other schools unless you apply early decision. My interviewer asked where I applied and I told him. He knew from my essay I wanted a rural location and in fact lauged at me when I said I applied to CCOM. He said, "oh yes, Chicago is definitly rural." I told him, rural is my first choice, but I was also applying to places where my husband could find a job more easily. And then we changed the topic. So my opinion, if they ask, tell them in a matter of fact tone and move on.
 
I don't think it hurts you to let them know you have other interviews. I look at it as letting them know that other schools think you're a competitive applicant and they better make you an offer quick so you'll come. If a school decided to not accept me solely because I had interviews at other schools I probably wouldn't want to go there anyway. They know good applicants have interviews, they're probably more interested in where and why you applied.
