A 30 year old male with 1 year history of type 2 DM is received insulin treatment but he is still hyperglicemia. What will be his endogenous fasting insulin concentration (N=5-20 uU/mL)?
A. 1 uU/mL
B. 10 uU/mL
C. 20 uU/mL
D. 40 uU/mL
 
Not sure how long it takes to reach insulin exhaustion phase in type 2 DM, but given that he was receiving insulin to begin with, I would say he has exhausted his insulin production.

I think it's D. 1 year h/o DMII means he is most likely still just insulin resistant, not dependent. In which case, the pancreas is over producing insulin to try to compensate and bring down the blood glucose. The fact that he was administered insulin for hyperglycemia doesn't mean that he isn't still producing it endogenously.
