So, I’m extremely disappointed with my score. Got a 473 and want to know if internal medicine is still possible, as well as doing a fellowship afterwards.
Yes its possible. Fellowships are going to be of the weaker variety tho: Endocrine, Rhuem, Nephro etc. Honestly its not the score that will limit you the most, its the IM residency that you will get in with that score. However, IM places a big emphasis on Step II, so that could be a golden ticket for you if you wanted to go that route. I will add my disclaimers about good letters, aways, hopefully a letter from aways being important as well. So, I’m extremely disappointed with my score. Is Step 2 CK a must for matching IM post merger? Is a Low Comlex Level 1 and 2 and a low USMLE Step 1 sufficient to open any IM doors?Yes its possible. What if my rotation site has an in house IM residency and Cardio fellowship? If I perform well, could that be an eventual possibility?Yes its possible. Mercy, you guys want a Kevin Garnett meme? What if my rotation site has an in house IM residency and Cardio fellowship? If I perform well, could that be an eventual possibility?
Yes. IM is not competative at the bottom. Community IM programs with no in house fellowships are just looking for a pass. The problem is, those programs will have a hard time placing you in most fellowships other than the ones I listed originally. Your not going GI or Cards (most likely) from a community program that has no in house fellowships. So you have to be honest with yourself and decide if you still want IM if the only fellowship options are things like Nephrology, or Endo, or Rhuem etc.Is Step 2 CK a must for matching IM post merger? This is why I was highly considering busting my butt and take step 1 sometime next summer. I’ve read about students doing it, but I also see how it can be tough to juggle year three, level 2, etc. while preparing for step 1.Mercy, you guys want a Kevin Garnett meme?

On a serious note, yeah its possible, and having an in house fellowship is a big advantage and would drastically increase your chances. Time with a program usually lets them know if they want you. Is it gaurenteed? Nope. So I guess the real question is: Are you okay with IM if you can't do Cardiology?
