I was wondering if ACGME says anything about internal moonlighting. Specifically whether it's allowable for the same institution to pay residents in one specialty one rate for moonlight, and another specialty a different rate...

The ACGME has nothing to do with internal moonlighting, other than requiring that any internal moonlighting be reported as duty hours. Institutions can pay people whatever they want.
