OK, the title says it's an internship, but in fact it's far from real medical internship; you know, those volunteer-like internships that many pre-med kids do.

I have interview on Tuesday. They never told me what exactly I am going to do, so I can't really describe what I will do.

What are they most likely to ask me during the interview, other than "why do you want to do" and "what do you plan to gain"?

Maybe your other relevant experiences?

Are they going to be as serious as real job interview?
I hope they don't test me on HIPAA rules XD.

Seriously, its a volunteer gig. Relax, its nothing as big as you make it sound, and if you are stressing this much over it, you are just going to look weird.

Or maybe I m really stressed because this is my first time and I don't know a thing about this.

Look OP. You're going to be doing things the nurses don't wanna do aside from cleaning the patient (i.e. nothing gross.) You'll be getting water, changing linens, and wiping down beds, tables, etc. with disinfectant wipes. That's going to be 98% of the job. Occasionally, you'll get to shadow the physician/PA/NP on rotation. Make connections for LOR if you go the DO route and shadow a DO.

Literally all you have to do with HIPAA compliance is not talk about things that could give a patient's info away or identify him/her.

They just want to make sure you're not a creep - pedophile, serial killer, etc. Since it's a volunteer position, stakes aren't high and they have to screen those people out somehow. It's low-stress so don't worry about it much. They'll probably use the opportunity to explain the position to you and give you the chance to ask them questions. They might ask you a few related behavioral questions just to make sure you're not an immoral serial killer but just be yourself and you'll be fine.

P.S. I doubt they'll ask you HIPAA questions at a first interview - volunteers (if this is a hospital) must go through various training sessions in which HIPAA is covered.
