Do any of you guys utilize ipsilateral lung constraints during lung Ca planning? For example, I have a guy who has very bulky ipsilateral disease. We did a tenative plan that gives a mean lung dose of 18, V20 of 32%, and V5 of 65%. The ipsilateral lung V20 however is 80+%.
 
I think that sounds like a reasonable plan for a bulky lung cancer. I do not look at the ipsi lung dvhs in lung like I do in breast. Your high ipsi lung v20 is likely a sign of your excellent contralateral lung sparing which is probably a good thing.
